Health History Form

To make your visit to Jackson Dental Arts as time-saving for you as possible, we ask that you provide us with your health history on the form found here (click on the link below to access the form). Simply fill in the form online, save it as a file on your computer and Email it to: judy@jacksondentalarts.com.
You can also print a copy of the filled in form (2 pages), sign it, and bring it with you on your first visit.
